  • Lack of Feedback – When reports disappear into a system with no visible outcome, people stop reporting.

  • Focus on Potential, Not Outcome – A near miss involving a flammable leak should be treated seriously even if it caused no harm because of what could have happened.
  • Identify Failed Barriers – Consider Which safeguards were supposed to prevent this and Why did they fail or prove insufficient?
  • Look Beyond Immediate Causes – avoid superficial conclusions like “operator error.” Instead, explore other causes such as System design, workload and fatigue, training adequacy and organisational influences.   • Share Lessons Broadly – Near misses should not stay within one team or site. Sharing across the organisation multiplies their value.

Near Misses and Process Safety

Near misses are particularly critical in process safety, where consequences are low-frequency but high-impact.

Unlike slips, trips, and falls, process safety events:

  • Can escalate rapidly
  • Often involve complex systems
  • Have catastrophic potential

A small deviation – such as:

  • A pressure relief valve lifting unexpectedly
  • A temporary loss of containment
  • A control system override

…may seem manageable in isolation but could signal a systemic weakness capable of leading to disaster.
