Risk assessments lie at the heart of modern safety management, yet catastrophic industrial accidents continue to occur in organisations that are demonstrably competent, well-resourced and compliant with prevailing regulatory frameworks. A recurring explanation for such failures is that the precipitating events were ‘unforeseeable’, or what Nassim Nicholas Taleb famously termed ‘Black Swans’, those rare, high‑impact events that fall outside normal expectations and are only rendered explicable in hindsight.

Many major industrial accidents are retrospectively framed as Black Swans not because they were genuinely unknowable, but because traditional approaches to risk assessment are poorly equipped to deal with uncertainty, complexity and cognitive bias. In particular, hindsight bias and the over‑reliance on historical precedent foster the dangerous assumption that ‘it has never happened before’, thereby constraining forward thinking. Taleb defines a Black Swan as an event that satisfies three criteria: it is an outlier beyond regular expectations, it carries an extreme impact, and it is subject to retrospective rationalisation that makes it appear predictable after the fact.

While the term originated in financial risk, it has been widely adopted in safety‑critical industries to describe disasters such as Piper Alpha, Deepwater Horizon and Fukushima Daiichi. In each case, the eventual chain of causation has been reconstructed in forensic detail, creating the impression that the accident was obvious and therefore preventable. Yet this impression is itself a cognitive illusion: as Taleb notes, our minds are highly effective ‘explanation machines’, adept at weaving coherent narratives after events have occurred.

The Piper Alpha disaster of 1988, which claimed 167 lives, is often cited as a case in point. Lord Cullen’s inquiry identified multiple systemic failures, including inadequate permit‑to‑work controls, flawed design assumptions and ineffective emergency response arrangements. With the benefit of hindsight, the vulnerabilities appear stark. Yet prior to the accident, the platform was regarded as a mature and well‑managed asset, and the specific combination of maintenance error, gas release, ignition and escalation had not been fully envisaged within prevailing risk models. Similarly, the Deepwater Horizon blowout in 2010 involved a cascade of technical and organisational failures across multiple contractors, culminating in an uncontrolled release from the Macondo well. The US National Commission concluded that the disaster was not the result of a single decision, action, or failure, but a series of failures that combined to overwhelm the safeguards. Such interactions are precisely the kinds of emergent factors that evade conventional risk assessment methodology.

A key reason these events are subsequently framed as Black Swans lies in hindsight bias. Hindsight bias refers to the tendency to believe, after an event has occurred, that one would have predicted or expected it beforehand, the familiar ‘I‑knew‑it‑all‑along’ effect. In the context of accident investigation, hindsight bias can distort learning by exaggerating the foreseeability of the outcome and underestimating the uncertainty faced by decision‑makers at the time. Dekker has argued that this creates a moral asymmetry, in which past actors are judged against knowledge that was unavailable to them, encouraging simplistic stories about ‘missed warning signs’ and ‘obvious’ errors. This not only misrepresents reality but risks reinforcing the belief that future accidents will be avoided simply by trying harder to spot what, in truth, only appears obvious with retrospect.

Traditional risk assessment methodologies exacerbate this problem by privileging historical data and probabilistic reasoning. Risk is commonly operationalised as a function of likelihood and consequence, with likelihood often inferred from past operating frequencies. This approach implicitly assumes that the future will resemble the past, an assumption Taleb criticises through the ‘turkey problem’: the turkey infers safety from a long history of being fed, only to be slaughtered at the point of maximum confidence. In industrial systems characterised by tight coupling and complex interactions, rare events may dominate risk yet remain statistically invisible until they occur. As recent work on low‑probability, high‑consequence events has emphasised, reliance on historical data becomes increasingly fragile in new or rapidly changing operating environments.

Importantly, several scholars and institutions have questioned whether so‑called Black Swan accidents are truly unforeseeable. Studies of natural‑hazard‑triggered technological accidents (Natech events) suggest that many could have been anticipated using existing scientific knowledge but were excluded from risk assessments because they lay outside design assumptions or regulatory scope. A recent review by the European Commission’s Joint Research Centre concluded that most recorded Natech accidents could not meaningfully be described as Black Swans, because relevant hazard information and experience already existed. From this perspective, Black Swans in industrial safety are less about inherent unpredictability and more about organisational blind spots and risk blindness. Disasters occur not simply because information is unavailable, but because it is discounted, normalised or rendered invisible by prevailing assumptions, commercial pressures or fragmented governance.

Overcoming the trap of ‘that’s never happened before’ therefore requires a fundamental shift in how risk assessment is conceived and practiced. One important development is the explicit consideration of uncertainty, rather than its concealment within numerical likelihoods that appear more precise than the underlying knowledge justifies. The latest thinking in risk management argues for distinguishing more clearly between risk and uncertainty, emphasising the strength and limitations of the knowledge that underpin risk judgements and making ignorance visible rather than implicit. By acknowledging what is not known, risk assessments can better inform decision‑makers about the limits of confidence, rather than presenting a false sense of completeness or control.

Closely related to this is the contribution of High Reliability Organisation (HRO) theory. Research into organisations such as air traffic control centres, nuclear power operations and aircraft carriers highlights the importance of ‘collective mindfulness’, a sustained preoccupation with failure and reluctance to simplify interpretations. Weick and Sutcliffe argue that HROs treat success as provisional and actively search for weak signals that may indicate emerging risk, even when everything appears to be going well. Rather than assuming that absence of accidents implies safety, HROs question routine assumptions, remain sensitive to operations and defer to expertise closest to the work. This mindset directly challenges the complacency embedded in the phrase ‘it’s never happened before’, replacing it with the more uncomfortable but more useful question: ‘what would surprise us most?’

Resilience engineering further extends this thinking by shifting attention from preventing specific failures to enhancing the system’s capacity to adapt and respond to the unexpected. Hollnagel’s Safety‑II perspective contrasts traditional safety management, focused on preventing things from going wrong, with an approach that also seeks to understand and support how things usually go right in everyday work.

From this angle, resilient systems are characterised by their ability to anticipate, monitor, respond and learn across a wide range of conditions, rather than by their adherence to a fixed set of controls. The practical implication is that the goal of risk assessment is not to identify every conceivable accident scenario, but to understand system behaviour under stress and to design margins, buffers and adaptive capacity that help the organisation absorb and recover from surprise.

Practical techniques can support this broader conceptual shift. Scenario‑based approaches that deliberately explore implausible or extreme conditions can help counteract the normalisation of risk and the tendency to anchor on the familiar. Techniques such as ‘what‑if’ analysis, red‑teaming and imaginative stress testing encourage assessors to step outside routine assumptions and challenge the boundaries of credible failure. These methods are particularly powerful when they draw on diverse expertise, including frontline workers who possess tacit knowledge of how work is actually done, rather than how it is imagined in procedures. Risk rarely exists only where the procedure says it is. It is where workers must make trade‑offs under pressure, in situations that are overlooked or only partially understood.

Many catastrophic industrial accidents retrospectively described as Black Swans are therefore not the product of pure randomness, but of constrained imagination, cognitive bias and methodological limitations in risk assessment. Hindsight bias creates the illusion that such events were obvious, while traditional probabilistic models can obscure uncertainty and reinforce reliance on historical performance. By putting uncertainty at the centre of our thinking, and by cultivating organisational mindfulness and resilience, safety practitioners can move beyond the comforting but hazardous assumption that the absence of past failure equates to future safety.
